“Full mouth” usually means rehabilitating both the upper and lower arches. All-on-4 and All-on-6 describe a fixed implant-supported prosthesis for one arch. If you need both, each jaw is planned separately. An assessment lets you compare implant numbers, prosthetic materials, treatment phases and costs per arch. We can also assess strategic corticobasal implants, even if you have sufficient bone, if you would like to explore a shorter timeline to the first fixed prosthesis.
Which option fits your arch?
All-on-4 supports a full-arch prosthesis on four implants; All-on-6 uses six. The number alone does not determine which is right for you. A 3D CT scan helps assess available bone and implant positions. Your bite, general health and prosthetic material also matter. In our conventional protocol, acrylic is often considered with All-on-4 and zirconia with All-on-6, but this is not a universal rule.
If some teeth may be restored, the team will explain those alternatives before recommending a full-arch replacement. When All-on-4 or All-on-6 is selected for an arch, remaining teeth in that arch are removed as part of the plan.
Conventional implants
The plan includes indicated extractions, implant placement and a transitional prosthesis. The definitive fixed prosthesis follows osseointegration. An immediately loaded fixed provisional can only be decided after implant stability and torque are measured during surgery. If they are insufficient, a transitional removable prosthesis may be considered when healing allows.
When bone is limited
CT imaging may show that a conventional plan needs grafting, bone expansion or sinus elevation. The team can also assess whether strategic corticobasal implants offer another route. These are different protocols, and the decision depends on your clinical findings.
Strategic corticobasal implants in Bogotá
Our team considers this a distinct fixed full-arch option alongside conventional All-on-4 or All-on-6. It seeks support from available cortical bone and, in our protocol, does not require prior bone grafting. It can be assessed for patients with bone loss and for those who have enough bone but want to compare a faster treatment sequence. Imaging, bite and medical history guide the individual plan.
First fixed prosthesis within 48–72 hours
For a planned strategic full-arch rehabilitation, connecting the implants with an initial fixed prosthesis in this period is part of the protocol. The acrylic option delivers a fixed acrylic prosthesis directly, without a transitional PMMA prosthesis. If definitive zirconia is selected, a fixed PMMA prosthesis is placed first; zirconia follows from eight months onward after assessing tissue healing.
Surgical approach and recovery
Our strategic approach aims to avoid raising broad tissue flaps when the anatomy and necessary extractions allow it. This may reduce tissue trauma, swelling and bruising, though it remains surgery and recovery differs by patient. After the fixed prosthesis is placed, patients may begin eating according to the prescribed diet and follow-up plan; unrestricted chewing is not promised immediately.
Conventional All-on-4 or All-on-6 may also allow immediate fixed loading when the required stability and torque are confirmed during surgery. No technique is automatically best for everyone. The assessment compares indications, risks, maintenance and preferences. Zygomatic implants are reserved for very severe upper-jaw atrophy when corticobasal support alone is insufficient.
Visits, treatment phases and costs
A conventional plan may take two visits when implants can be placed during the first surgery. If grafting must happen first, three visits may be needed. The definitive prosthesis is planned after integration; timing depends on grafting and healing. The cost is divided between the initial and definitive phases, so the full amount is not due at the start.

Will the first appointment confirm that I need All-on-4?
Not necessarily. The assessment covers teeth, gums, general health, bone and bite. Another type of rehabilitation or implant technique may be more appropriate.
Can I receive fixed teeth on the day of surgery?
With conventional implants, an immediately loaded fixed provisional depends on implant torque and stability confirmed during surgery. If the requirements are not met, the team will explain the transitional option and aftercare.
